Your first visit is all about you: your comfort, your happiness, and your health
The first visit to our Flagstaff dental office is designed to get you better acquainted with all we offer, as well as to introduce you to Dr. Geisel and our caring staff. We encourage questions and do our best to deliver quality care always.
Please take a moment prior to your scheduled appointment to fill out the online patient forms through the above link. We ask that you complete the forms and bring them to your appointment so we may assist you better and in a timely manner. Thank you for your confidence in our office. If you have been seen within the last five years and have had X-rays taken, it is important for us to receive copies of these from your previous dentist.
If they are of good quality, it may not be necessary to have them taken again at your first visit here. Please complete a “records release” form and return it to our office. If no X-rays have been taken, or if the ones forwarded to us are not of sufficiently good quality, we will need to take diagnostic films at your first visit.
Some dental insurance plans have limits on their coverage of radiographs/x-rays. If you have dental insurance, please check with your policy’s coverage. You might be responsible for the fees, if insurance may not cover them. Please call the office if you have any questions regarding our X-ray policy.

The American Dental Association recommends a visit to your dentist every six months for regular checkups and routine teeth cleanings to keep your teeth and gums healthy.
We understand you have a busy schedule, so when you’re preparing to visit our office, please remember your appointments can last between 45 and 90 minutes.
